Opens on May 1, 2024. This form is required if you have comparable health insurance coverage and want to opt out of the Reed plan. If you do not submit a waiver, you will be charged for Reed's health insurance. - Health Record and Vaccine Proof*
Review Reed's immunization requirements and health record requirements. Then, complete required health forms through the Student Health Portal and upload proof of required vaccines or exemptions (Reed requires proof of COVID vaccine primary series/booster and MMR series). - Housing: Residence Life Forms*
Opens on May 1, 2024. Required for all incoming first-year students and transfers covered under our residency requirements. For more information about living on campus, visit the residence life website. - Disability Accommodation Request Form (Housing)
Opens on May 1, 2024. Complete to request accommodations for housing. DAR can accept housing accommodations requests past June 1st, however, after that date ability to implement accommodations is dependent on the available housing inventory. - Orientation Programs and Experiences: Peer Mentor Program
Applications opens on May 1. Reed's Multicultural Resource Center's Peer Mentor Program is designed to foster a sense of belonging and support among historically underrepresented students and introduce those in their first year at Reed to valuable campus resources and relationships. Apply if you are interested in being matched with a peer mentor for your first year at Reed. The deadline to apply is extended until June 15. - Photograph Submission for Reed ID*
Upload your photo for your Reed ID card. This is required for move-in. - Second Enrollment Deposit*
Your first enrollment deposit was submitted when you accepted Reed’s offer of admission. - Social Security or Tax ID Number*
Federal regulations require that students provide a Social Security number or Tax ID so the college can issue a 1098-T tax form. See IRS Publication 970 for more information. If you do not have a Social Security number or Tax ID, or do not wish to provide it, indicate that on this form. - Statement of Financial Responsibility*
Outlines the student’s personal responsibility for college charges assessed to their account. - Student Profile and Contact Information*
In IRIS, update your personal information (including your preferred first name, gender, pronouns, and orientation t-shirt size) and contact information (including your mailing address, phone number) and emergency contacts. - Tuition Insurance Refund Plan Waiver
This form is required if you want to opt out of tuition insurance, which covers 70 percent of the term’s tuition and fees if you withdraw for medical reasons. Learn more about tuition insurance on our website.
After you accept admission to Reed and submit the first deposit, Reed will send you an email with information and instructions for setting up your computer account and email.
